NPRC matches records by name + date of birth + last 4 of SSN. If you're filling this in for a family member and don't know the exact date, give your best estimate — they can usually find the file with the name and an approximate DOB.

Where does the full SSN go?

It doesn't go anywhere on this site. NPRC needs the full number on the SF-180, but we only collect it later through our encrypted signing partner — the full SSN never touches our database or shows up in any email.
